Dr. Montañez provides psychological services to individuals with Washington State workers' compensation claims through the Department of Labor & Industries (L&I).

 

How to Make a Referral

 

We welcome referrals from physicians, psychologists, counselors, social workers, and other professionals involved in the care of individuals with L&I claims.

Please fax the referral and relevant supporting documentation to the secure fax number above.

 

Our electronic fax service is configured for the secure transmission of protected health information (PHI) and is supported by appropriate HIPAA safeguards.

 

For patient referrals and clinical records, please use the secure fax rather than the website contact form or ordinary email.

 

Please verify the fax number before transmitting confidential patient information.

 

Information to Include

 

To help us review and process the referral, please include as much of the following information as is available:

 

• Patient's full name and date of birth
• Patient's telephone number and other relevant contact information
• L&I claim number
• Date of injury, if known
• Referring/attending provider's name, practice, telephone number, and fax number
• Reason for referral and relevant clinical information
• Relevant medical and behavioral health records
• Current treatment information and medications, when relevant
• Relevant L&I claim or authorization documentation
• Any specific questions or concerns you would like adressed
• Your contact information and preferred method for follow-up

 

Please include the patient's L&I claim number whenever available so that we can more efficiently identify and process the referral.

 

What Happens After a Referral?

 

After receiving a referral, Dr. Montañez will review the information provided and determine whether the referral is appropriate for her practice and services.

 

If additional information is needed, we will contact the referring provider.

 

Please note that referral submission does not guarantee acceptance of a referral or availability for an appointment. Services are subject to applicable L&I requirements, claim status, authorization requirements, clinical appropriateness, and scheduling availability.
